The policy also provides for additional benefits such as the Grocery Benefit up to K6, 000.00 plus Education Benefit up to K5, 000.00 on death of main member in addition to the main

What you get
  • The Policy holder
    • under 65 years (inclusive) at commencement date
  • One spouse
    • either by civil, tribal, common or customary law
    • under 65 years (inclusive) at commencement date.
    • only one spouse is eligible for the spouses benefit.
  • A maximum of 6 children will be eligible for cover
    • A legitimate child
    • An illegitimate child who is legally dependent on the Principal Life Assured
    • An adopted, foster or step child
    • A stillborn child who is legally dependent on the Principal Life Assured
    • An unmarried child of the Principal Life Assured up to and including the age of 21
    • A mentally or physically disabled child of any age who is fully dependent on the Principal Life Assured
  • A maximum of 4 parents (biological or legally adopted)
    • Under the age of 75 (inclusive) at the date they are added
  • A maximum of 6 other dependant relatives.
    • A relative of the Principal Life Assured.
    • Under the age of 65 at the date they are added

 

  • Premium Waiver on Death of Main member is applicable to immediate family and remains in force for a period of 12 months. Policy must have been in force for a period of 6 months. After the 12 month period the premium needs to be paid.
  • Premium waiver in event of retrenchment of Main Member: In the event of main member being retrenched, before the age of 65, the premium will be waived for 3 months. Policy needs to have been in force for 12 months. Only applicable to premiums to cover immediate family.

Visit your nearest branch with the following documents and fill in an application form:

  • Copy of your identity document (NRC or passport)
  • Two passport-size photographs

Proof of address - utility bill less than 3 months old

Claim requirements

The following claims documents will be required when submitting a claim:

  • Either a Certified copy of Death certificate or Burial Order or
  • Police report if death by accident or
  • Medical certificate of cause/s of death
  • One of the following Certified copy of Identity/birth certificate of the deceased
  • Zambian Identity Document or
    • Valid Passport or
    • Valid Driver’s Licence
  • One of the following Certified copy of the Identity of the claimant
    • Identity Document or
    • Valid Passport or
    • Valid Driver’s Licence
